ASSUMPTION OF RISK: I understand that participation in activities at The Play Station involves inherent risks, including but not limited to physical injury from falls, collisions with other participants or equipment, and other accidents.
RELEASE OF LIABILITY: In consideration for being permitted to use The Play Station facilities, I hereby release, waive, and discharge The Play Station, its owners, operators, employees, and agents from any and all liability, claims, demands, and causes of action arising out of or related to any injury sustained while participating in activities.
RULES AGREEMENT: I agree to ensure my child(ren) follow all posted rules and staff instructions. I understand that socks are required and I am responsible for supervising my child(ren) at all times.
PHOTO/VIDEO CONSENT: I grant The Play Station permission to use photographs or video taken of my child(ren) for promotional purposes.
